New Patient Enrollment

Our enrollment process includes a six-week Initial Assessment and Onboarding Period which consists of scheduled appointments and touch points between appointments.  This structured approach ensures continuous progress toward a plan that can support lasting health improvements. Dr. Luetmer works with individuals, couples, and families, adjusting our onboarding processes below to accommodate the specific needs of her patients.

Becoming A Patient

Since starting Flourish Health in 2019, we have continually fine-tuned the new patient onboarding process with the goal of achieving the best possible results while containing costs.

There are two phases to becoming a patient of Flourish Health. The first phase is a 6-week onboarding process that has a fixed number of visits with support between visits. By having a structured onboarding period, you and Dr. Luetmer will have sufficient time to “dig in” to your health history and formulate a health plan going forward.

At the conclusion of phase one, If you feel you have made good progress and want to continue working with Dr. Luetmer, you may either join the practice as a member patient or choose to see Dr. Luetmer at her hourly rate as needed.

Phase 1: The Initial Assessment & Onboarding

This phase lasts 6 weeks beginning with the first phone appointment upon completion of your Case Review. The fee for Phase 1 is $950 for an individual or $1,600 for a couple. 

Appointment times for couples are lengthened from what is listed below. No fees are incurred until after you submit your case review and we mutually agree that Flourish Health is a good fit for you.

The Readiness Assessment and Case Review: You will start by taking a short readiness assessment. If you have not already completed this, the link is above. The readiness assessment is designed to measure your readiness to make lifestyle changes that may be necessary to achieve your health goals. This will be followed by completing a comprehensive medical history and self-assessment, which we refer to as the case review. The case review will likely be much more thorough and unlike those you may have completed in the past and is designed to dig below the surface so you can work with Dr. Luetmer to identify the root cause of your health issues.

Portal Access

Once you complete the case review, we will take care of the administrative tasks of signing consent forms and providing you with access to the Member Portal. Dr. Luetmer will review your case prior to your initial phone appointment to confirm that she believes there is a fit between your needs and her qualifications and the nature of her practice. If she does not think that she can be of help, we will let you know and you will have no financial obligation for her time spent on your case review and any payment made to that point will be refunded.

Six-Week Onboarding Timeline

Phase 2: Annual Membership or Hourly Consulting

The journey to sustainable good health is a process that requires ongoing effort and commitment. Membership is designed to provide the ongoing support, educational resources, and networking with other members to increase your odds of long-term success.

At the conclusion of Phase 1, you will receive a recommendation for the appropriate membership level (see options below) based on Dr. Luetmer’s assessment of your treatment plan progress and support ticket activity.

You are not obligated to have an annual membership with Flourish Health in order to continue working with Dr. Luetmer. Some patients opt to forego member benefits and see Dr. Luetmer as needed at her hourly rate.

Membership Has Its Privileges

Base Membership

Base membership benefits are included as part of all Full Membership Plans (Bronze, Silver, Gold, & Platinum). Base membership is also available as a stand-alone benefit ($65/month).

At Flourish Health, we spend a great deal of time and energy negotiating the best price on health services and products on behalf of our members and those savings are passed on to you. Members enjoy perks such as discounted pricing on labs and reduced prices on supplements.

Members also receive access to  online education courses, group meetings, and other benefits.

Full Membership

In addition to the benefits of the base membership, patients with full membership also have access to Dr. Luetmer and our health coaches throughout the year via in-person visits as well as telephone and Zoom (virtual) visits.

Members also have access to our member portal through which they can securely communicate their health questions and retain a history of all their communication with Dr. Luetmer for future reference. To hear what this access means to current patients, click here for testimonials.

Membership Plans

The anticipated medical needs of the individual, couple, or family beyond the onboarding process and the degree of ongoing support you are seeking will determine your level of annual membership. Ongoing support includes a combination of in-person visits, virtual visits, group visits, and messaging communication provided by Dr. Luetmer and our health coaches through our portal. While most patients who see Dr. Luetmer value the benefits of membership, it is not required that you become a member. Some patients see Dr. Luetmer at her hourly rate as needed.

This membership level is designed for individuals who have up to one moderately complex chronic condition that requires intermittant visits throughout the year and a modest amount of ongoing support.

This membership level is designed for individuals who have one or more moderately complex chronic condition(s) that require a relatively consistent amount of ongoing support. Additional time is built in for more clinic visits. Couples with only one spouse experiencing complex issue(s) and relatively healthy families may also fit into this plan.

​This membership level is designed for individuals who have complex health issues and/or desire a higher level of ongoing support. Frequent touch points are built in to accommodate a close partnership with Dr. Luetmer to avoid cycling through major health swings. Complex couples and larger families with one or more family members facing complex issues will also fit into this plan.

Platinum Members receive all Gold benefits as well as access to Dr. Luetmer’s personal mobile number.
This membership level is designed for individuals who have multiple and very complex health issues and/or desire unparalleled access to a functional medicine MD. Due to the nature of this membership, the Platinum level is limited to 10 patients. ​

What are Member Credits?

Membership credits primarily translate to consulting time you can “spend” as part of your membership and provide you with the flexibility to connect with Dr. Luetmer and our health coaches, using the avenue that works best for you.

For example, some people prefer using most of their time for in-person appointments; whereas others like to spend more of their credits toward online support to ask ongoing questions of Dr. Luetmer or our health coaches.

1 credit equals 1 hour of consulting time with Dr. Luetmer, whereas 1 credit equals 2 hours of consulting time with Michelle, our health coach.

How do I Maximize my ​Credits?

There are a number of member benefits that do not reduce your credit balance; from member group events to live Q&A educational seminars.

In addition, participating in group clinic visits to discuss health issues and strategies common to the group will only deduct a fraction of a credit.

For example, group visits that generally last between 60-90 minutes only “cost” .25 (¼) of a credit. A unique aspect of Dr. Luetmer’s practice is the vast library of video education that is available to members only.

This education is designed to elevate a member’s understanding of their health issues. Dr. Luetmer “prescribes” specific videos to patients so members can maximize their appointment time since Dr. Luetmer is not wasting time “covering the basics” that are addressed in the video education.
